Registration With The Practice
If you wish to register with the Practice you will need to submit an application, you can do this by telephone or calling into reception.
You will receive a letter if your registration has been approved.
If your registration is approved there are documents and forms (which are available at reception) required in order for your registration to be processed.
All documents and forms must be submitted before we can process your registration.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
